1

Go to the List menu at the top, select the e-mail list you would like to include.

2

Go to the Signup forms menu and click on the Select button at the Embedded forms.

There will be a Copy/paste onto your site area

So copy the content from there.

Open some file editor and paste the code there, then look at the code. It will be something like this:

<!-- Begin MailChimp Signup Form -->
<link href="//cdn-images.mailchimp.com/embedcode/classic-081711.css" rel="stylesheet" type="text/css">
<style type="text/css">
	#mc_embed_signup{background:#fff; clear:left; font:14px Helvetica,Arial,sans-serif; }
	/* Add your own MailChimp form style overrides in your site stylesheet or in this style block.
	   We recommend moving this block and the preceding CSS link to the HEAD of your HTML file. */
</style>
<div id="mc_embed_signup">

    <div id="mc_embed_signup_scroll">
	<h2>Subscribe to our mailing list</h2>

<!--End mc_embed_signup-->

Copy the link and the style tags into your theme's header.php or into your template's index.php file right before the closing head tag:

<link href="//cdn-images.mailchimp.com/embedcode/classic-081711.css" rel="stylesheet" type="text/css">
<style type="text/css">
	#mc_embed_signup{background:#fff; clear:left; font:14px Helvetica,Arial,sans-serif; }
	/* Add your own MailChimp form style overrides in your site stylesheet or in this style block.
	   We recommend moving this block and the preceding CSS link to the HEAD of your HTML file. */
</style>
</head>

Go to the slide where you'd like to have the form, put down a HTML layer, and paste the rest of content there to the HTML field.

You might put your CSS codes to the CSS field of the HTML layer if needed.
